To become a new patient, please print out and read the four intake forms. Please make an appointment for your first visit using the online system below (choose "1st Session"). Bring your completed intake forms with you to your first visit.
If you would like to use Cigna, New Mexico Health Connections or Presbyterian insurance, when making your appointment, please call 505.690.8048 to provide the name of the insurance company & plan, your member number or ID number, date of birth and legal name and we will verify your benefits, copay and deductible. We can bill BCBS and United if your plan has out of network acupuncture benifits. Some supplemental Medicare packages include acupuncture benefits.
To make an appointment, you can schedule online at your convenience for faster and easier scheduling 24 hours a day. If you include your cell number and email address, you will automatically receive an appointment reminder 28 hours in advance and you can pay online, or at your session with cash, credit card or check. Being in Santa Fe, it’s very hard to stay hydrated, especially when you are pregnant and when you are breastfeeding.
